Internship Commitment Form Overview

Registration Deadlines – Please make sure all required registration paperwork is completed at least a week before the deadline in case there are any issues that need addressed.

Before completing the Internship Commitment Form (ICF):

  • Faculty Sponsor – Seek approval of the internship from the faculty member who has agreed to sponsor your internship (see Faculty Sponsors on the internship website) and discuss your academic assignments with them. You will need to list their contact information on the ICF so they can approve it.
  • Faculty Academic Advisor – You will need to list your academic advisor’s contact information on the ICF so they can approve it.
  • Site Supervisor – Discuss the duties of the internship with the employer who will directly supervise you on site.

Other Important Information:

  • You may SUBMIT THE ONLINE INTERNSHIP COMMITMENT FORM ONLY ONCE per semester so make sure the information is correct (double check spelling, email addresses, etc)
    • An INCORRECT EMAIL ADDRESS means your intended party will not be able to approve your ICF which will slow the approval process down.
  • READ YOUR EMAILS for further instructions on how to register for credit once your ICF has been submitted. You will be sent an email informing you that all signatures have been completed and what to do to register for credit.

  • Your Personal Information
  • Course Information (semester, major/minor you are seeking credit for)
  • Faculty Sponsor Information
  • Faculty Academic Advisor Information
  • Site Supervisor Information
  • Company Information
  • Internship Description
  • Learning Objectives (What do you hope to gain from the experience?)
